Early diagnosis and management are essential to reduce the morbidity in neonates with liver diseases. The manifestations of liver diseases in neonates may differ from those in older children, and the most common presentation in neonates is cholestasis, irrespective of the underlying cause. Several disorders unique to neonates include cholestatic disorders, vascular abnormalities, tumors, and acquired conditions. Radiologists need to be familiar with these etiologies as imaging has an important role to play in the diagnostic armamentarium, and early diagnosis is essential to reduce/prevent morbidity and mortality. Ultrasound (US) examination is the initial screening investigation for hepatobiliary pathologies in neonates. It is a quick, portable, and radiation-free modality, and thus, it is beneficial in the initial diagnostic workup.  This chapter reviews the spectrum of hepatobiliary conditions that manifest in neonates and will help a general radiologist make the diagnosis of liver and gall bladder pathologies in neonatal population. Various categories of abnormalities that will be described include cholestatic disorders, tumors, vascular abnormalities, and a few other acquired miscellaneous conditions.
