The setting up of a cancer center, of which a radiotherapy facility is an integral part, needs wide coverage. Such a center shall have multidisciplinary cancer care facilities, appropriate infrastructure, human resources, and equipment needed for various services, all of which require in-depth analysis and presentation of various facts. The recent World Health Organization (WHO)–International Atomic Energy Agency (IAEA) joint publications titled “Setting up a cancer center: A WHO-IAEA framework” [1] and “The Comprehensive Cancer Center—Development, Integration and Implementation” by Aljurf et al. [2] form ideal references in this regard. Instead, this chapter will concentrate on how to set up a radiotherapy facility and related issues.
