Radiation therapy with its local as well as proposed systemic effects can be a valuable addition to immunotherapy or targeted therapies. The synergistic effects of these therapies when used with radiation have been noted in both pre-clinical and clinical studies across various tumour types. In this chapter, we have described the biological mechanisms of combining immunotherapy with radiation and summarized the existing as well as ongoing clinical studies supporting it. In addition, we have proposed methods for optimizing this combination by altering parameters such as dose per fraction, type of radiation, timing of radiation, etc. Similarly, we also have addressed the rationale for combining targeted therapies with radiation, discussing ways to optimize the combination as well as the available evidence supporting or refuting it. Combination therapies with either of them with radiation therapy is a promising approach with strong biological rationale and emerging clinical evidence. It is vital that the future clinical trials unravel the ways to leverage this synergism for incremental patient benefits.
