Treatment of Erythropoietin-Induced Pure Red Cell Aplastic Anemia

ESA hyporesponsiveness makes it difficult for clinical treatment of renal anemia. In this case, we reported a 70-year-old male patient undergoing peritoneal dialysis with moderate to severe anemia. His hemoglobin gradually increased to relatively normal levels after receiving a high dose of EPO. However, severe anemia occurred again after 10 months’ treatment. On the basis of anemia recurrence and bone marrow aspiration results, the patient was diagnosed with EPO-induced PRCA and his medication regimen was switched from EPO to Roxadustat, along with cyclosporine and WuZhi capsules for immunotherapy, which effectively and safely corrected anemia. This case suggest that EPO-induced PRCA should be considered when progressive anemia occurs during the use of EPO, and Roxadustat with immunotherapy co-administration would be an effective treatment for these patients.
