Differential Diagnosis of Malignant Transformation of Mature Teratoma

A patient in her 50s visited our hospital due to a sensation of a lower abdominal mass. A transvaginal ultrasound revealed a pelvic mass larger than 10 cm, suspected to be a teratoma. If it were a benign mature teratoma, the patient would undergo laparoscopic removal of the affected adnexa. However, if it turned out to be a mature teratoma with malignant transformation, a laparotomy would be performed to remove the uterus and bilateral adnexa. So, the question arises: how can we differentiate between these scenarios on imaging?
