This chapter describes the pathology of glandular neoplasia of the uterine cervix. In modern series, most cervical adenocarcinomas develop as a result of human papillomavirus infection, but a significant minority are unrelated to it. When invasive, these HPV-independent glandular lesions have aggressive clinical behavior and are refractory to standard adjuvant treatments. The outline of this chapter follows the 2020 World Health Classification of Female Genital Tumours, in which the major categories are HPV-associated and HPV-independent adenocarcinoma (either in situ or invasive), with several subtypes or variants. As in the first edition of this book, we include discussion around the diagnosis of stromal invasion, staging, and pattern-based assessment (the latter applicable only to HPV-associated invasive endocervical adenocarcinoma).
