Interventional Treatment and Imaging Evaluation of Hepatobiliary Diseases

The chapter provides a comprehensive review of interventional treatments and imaging evaluations for hepatobiliary diseases, highlighting the significant role of interventional radiology. Interventional radiology is characterized by its minimally invasive nature, repeatability, accurate positioning, and high efficacy, making it one of the three major clinical medical technologies alongside internal medicine and surgery. Transcatheter arterial chemoembolization (TACE) is a pivotal treatment for hepatocellular carcinoma (HCC), particularly for patients who are not eligible for surgery. TACE involves infusing chemotherapy drugs and embolic substances into the tumor’s blood supply arteries to block blood flow and induce tumor necrosis. The chapter discusses in detail the principles, techniques, indications, contraindications, and clinical applications of TACE. It also explores its combination with other treatments such as radiofrequency ablation (RFA) and drug-eluting microspheres. Physical ablation techniques, especially RFA, are also addressed in this chapter. RFA is a minimally invasive treatment that uses imaging guidance to insert electrode needles into tumors to generate heat and destroy cancer cells. The chapter outlines the fundamental equipment requirements, principles of operation, indications, contraindications, and procedural methods for RFA while discussing its clinical efficacy and potential complications. Furthermore, the integration of advanced imaging modalities including ultrasound, CT, and MRI in guiding interventional procedures is emphasized. The utilization of these imaging techniques enhances intervention accuracy and effectiveness by providing real-time visualization of the tumor and surrounding structures.
