Atypical Femur Head Fracture Pattern: Management Issues

Most literature on femoral head fractures categorizes these injuries using the Pipkin classification. However, numerous atypical injury patterns that do not align with the traditional Pipkin classification have been documented. These injuries are clinically significant and have been incorporated into more recent classification systems. Due to the limited descriptions available, recognizing these uncommon injury patterns is crucial for defining appropriate management protocols. Associated injuries, such as acetabular fractures and posterior hip dislocations, are frequently encountered and play a critical role in refining the surgical approach to femoral head fractures. Notable patterns outside the Pipkin classification include isolated femoral neck fractures with posterior femoral head dislocation, femoral head fractures with anterior or medial hip dislocation, and impacted or sleeve fractures of the femoral head. These rare injury patterns often require specialized interventions. A comprehensive literature review was conducted to identify common atypical patterns and effectively guide subsequent stabilization strategies.
