Lipedema is a condition characterized by the accumulation of excess adipose tissue in the subcutaneous layer of the body, primarily in the lower extremities. The histopathology of lipedema tissue shows several distinct features, which can help to confirm the diagnosis. One of the key features of lipedema tissue is the presence of hypertrophied adipocytes, or fat cells. These cells are larger than normal and have a characteristic “signet-ring” appearance, with a small nucleus pushed to one side of the cell by a large lipid droplet. This is in contrast to normal adipocytes, which have a round nucleus and a smaller lipid droplet. In addition to the hypertrophied adipocytes, lipedema tissue may also show signs of inflammation and fibrosis. There may be an increase in the number of macrophages and other immune cells in the tissue, as well as fibrosis and thickening of the collagen fibers that support the adipose tissue. Lymphatic abnormalities are also commonly observed in lipedema tissue, including dilation of lymphatic vessels and lymphatic capillaries, and accumulation of lymphatic fluid in the interstitial spaces. These changes may contribute to the swelling and edema that are characteristic of the condition. Finally, studies have suggested that there may be alterations in the expression of certain genes in lipedema tissue, which could contribute to the development and progression of the condition. These include genes involved in adipocyte differentiation, inflammation, and lymphatic function.
