Photodynamic therapy (PDT) is an effective and safe treatment for various nonmelanoma skin cancers (NMSCs), particularly those recommended for actinic keratosis, squamous cell carcinoma in situ, and superficial basal cell carcinoma (BCC). However, its efficacy for nodular BCC, locally advanced BCC, and invasive cutaneous squamous cell carcinoma (cSCC) remains unsatisfactory because of the limited penetration depth of photosensitizers and light sources, which restricts broader applications. Recently, neoadjuvant PDT has emerged as a promising approach in NMSC management, offering the advantage of complementing mono-PDT treatment (Yan et al., Curr Treat Options Oncol 25(6):813–826, 2024). This chapter explores neoadjuvant PDT, covering its definition, therapeutic protocols, clinical applications, efficacy evaluation, adverse reactions, and patient management to support clinical practice.
