ALA-PDT in the Treatment of Skin Aging

In addition to nonmelanoma skin cancers and condyloma acuminatum, high-quality evidence strongly supports the use of photodynamic therapy (PDT) as an effective strategy for skin rejuvenation. PDT has the potential to slow the aging process by promoting fibroblast proliferation, stimulating keratinocyte–fibroblast interactions, reducing oxidative stress, preventing DNA damage, and remodeling collagen, lymphatic vessels, and the immune microenvironment in aging skin. Various light sources, such as light-emitting diodes, powerful pulsed light, pulsed dye lasers, and daylight, have been used in clinical studies and have shown promising results in reducing fine wrinkles, facial erythema, rough texture, and mottled pigmentation. Recent research suggests that combining multiple light sources sequentially can enhance cosmetic effects, although this is associated with increased costs. Adverse effects are typically mild to moderate, but further optimization of PDT protocols and a deeper understanding of their mechanisms are needed for skin rejuvenation.
