The role of pediatric ventricular assist devices (VADs) in the treatment of severe heart failure in children is expected to become increasingly important. Currently, the only durable VAD available for infants and small children is the Berlin Heart EXCOR®(EXCOR), and experience with its use is beginning to accumulate. In Japan, it has been about 10 years since the start of this pediatric VAD, and a total of 133 implantations were performed as of 2024. Out of these, 59 cases (44.4%) have reached heart transplantation. Thus, EXCOR has an exclusive role in pediatric heart failure treatment especially aiming for transplantation. At Osaka University, a total of 37 patients received EXCOR support and its demographics showed the age range from 4 months to 11 years, and the duration of support 45 to 1421 days. Among these, 15 patients have reached heart transplantation.
