This study aims to determine teacher performance in developing evaluation tools for physical education learning outcomes—a survey on the ability to formulate research planning and implementation. The study population consisted of 38 high school physical education teachers, 12 junior high school physical education teachers, and 33 elementary school physical education teachers in the Special Region of Yogyakarta. This study uses quantitative descriptive data analysis techniques with percentage data. Research shows that the performance of high school physical education teachers in planning and carrying out assessments is: sufficient. This can be seen from the average (mean) performance of physical education teachers in planning and carrying out evaluations = 36. Performance of junior high school physical education teachers in planning and implementing assessments: sufficient. This can be seen from the average (mean) performance of physical education teachers in planning and carrying out evaluations = 36. Performance of SD Physical Education teachers in planning and implementing assessments: Good. This can be seen from the average (mean) performance of physical education teachers in planning and implementing evaluations = 35. Based on the analysis of quantitative data on the questionnaire data, it can be concluded that in general the teacher's performance in compiling instruments for assessing student learning outcomes in physical education subjects in the Special Region of Yogyakarta is in the appropriate category.
