Cervical laminoplasty is often preferred over decompression laminectomy for multilevel cervical spondylotic myelopathy (CSM) due to its benefits in preventing restenosis and epidural adhesions. However, the endoscopic approach is typically reserved for decompression laminectomy due to technical challenges [1]. We attempted biportal endoscopic cervical open-door laminoplasty using “hooking technique” and customized instruments, successfully performing the procedure from the unilateral side [2]. The advantages of biportal endoscopic surgery, which allows independent two-handed manipulation, contribute to technical advancements, making endoscopic laminoplasty a viable option for treating CSM [1, 3, 4]. This technique represents a significant milestone in expanding the application of endoscopy for cervical degenerative diseases.
