Traditionally, migrated disc herniation within the neural foramen required an open extraforaminal approach to access the compressive disc fragment. The outcomes of this open approach are associated with limitations of access and dorsal root ganglion irritation. However, UBE (unilateral biportal endoscopy) allows for minimally invasive access to the foramen using an outside-in technique via a paraspinal approach. When a migrated disc fragment affects both the traversing and exiting nerve roots, a contralateral sublaminar approach can address these through a single procedure [1–5]. This chapter will summarize this approach.
