Finger Pain Beyond Stenosing Tenosynovitis

Hand injuries represent a common occurrence, particularly among athletes [1]. Among the myriad of injuries, involvement of the collateral ligaments of the metacarpophalangeal and interphalangeal joints is frequently encountered. Additionally, trigger finger [2], a pathological condition affecting the digital pulleys and flexor tendons, presents a notable challenge. Preceded by pain and joint stiffness, the triggering phenomenon adds complexity to its diagnosis and management. Furthermore, in patients with rheumatoid arthritis, tendon tears can lead to various hand deformities, including the Boutonniere deformity [3]. Another well-known condition, “Boxer’s knuckle” [4], prevalent among boxers, typically results from clenched-fist trauma, primarily affecting the third metacarpal head. Its pathology involves tears of the sagittal bands of the dorsal hood and associated extensor tendon instability. Given the diverse nature of finger disorders, precise imaging diagnosis becomes imperative. Capitalizing on its superior visualization of superficial structures, ultrasound has emerged as a preferred modality for diagnosing finger disorders. This chapter aims to explore the sonoanatomy of the extensor/flexor tendons of the fingers, along with the associated connective tissue structures such as the sagittal band and annular pulley. Additionally, common pathologies like trigger finger will be elucidated through ultrasound findings, along with insights into ultrasound-guided injection techniques for its management.
