Many minimally invasive methods have been described to treat pilonidal sinus disease. None of them ensures complete removal of hair and debris under vision. Meinero and Milone described endoscopic method (EPSiT/VAAPS) to manage pilonidal sinus disease under vision (Meinero et al, Color Dis 18(5):O164–O170. https://doi.org/10.1111/codi.13322 . Indications and contraindications of endoscopic method are described and shown with clinical photographs as we observed over the years. Operative steps of the procedure as we follow today, are elaborated with operative photographs and videos. Care of the scope and delicate endo-instruments is stressed upon. Methods to use LASER, instead of monopolar current, are explained. “Can we combine endoscopic method with a small flap for benefit of the patient?” Is a food for thought and have been demonstrated with clinical photographs of a young lady.
