A 5-year-old boy suspected of idiopathic pulmonary arterial hypertension due to a significant decrease in exercise tolerance was scheduled for cardiac catheterization under general anesthesia. Young age, general anesthesia, pulmonary artery pressure higher than aortic pressure, and heart failure can all trigger intraoperative pulmonary hypertension crisis. Preoperative thorough assessment and avoidance of intraoperative hypoxia, acidosis, and hypotension are key to the successful anesthetic management of cardiac catheterization for this kind of case.
