A 7-day-old female infant with transposition of the great arteries underwent emergency laparotomy for intestinal perforation and septic shock under general anesthesia. The anesthesia for this case was extremely challenging. It is necessary to make a detailed anesthetic plan in advance based on the physiological characteristics of the newborn period, the pathophysiology of transposition of the great arteries, and the principles of septic shock treatment. Among them, the key points of perioperative anesthetic management are adjusting the systemic and pulmonary vascular resistance to improve tissue organ perfusion, rational use of vasoactive drugs, and optimal fluid management.
