Anesthetic Management of Right Ventricular Outflow Tract Obstruction Relieving Surgery in a Child with Right Ventricular Myocardial Hemangioma

A 6-year-old boy was found to have a heart murmur during a clinic visit for a fever. Echocardiography indicated a mass in the right ventricle (possible tumor) and right ventricular outflow tract obstruction (RVOTO). He was scheduled for cardiac tumor resection and right ventricular outflow tract (RVOT) relieving surgery. The postoperative pathology result indicated myocardial hemangioma. RVOTO can increase the right ventricular afterload and cause secondary myocardial hypertrophy and cardiac dysfunction. Severe RVOTO can result in hypoxemia due to reduced pulmonary blood flow. The anesthetic management of such children should focus on reducing the right ventricular afterload, maintaining sufficient preload, and preserving cardiac function.
