The endothelial cell layer of the cornea lines the inner surface of the cornea and regulates corneal stromal hydration. This single, approximately 5-μm thick layer of polygonal cells serves as a passive barrier to imbibing fluid from the aqueous humor and actively pumping fluid out of the cornea. Endothelial dysfunction or damage results in corneal edema, reducing corneal transparency. This can lead to painful blistering of the corneal epithelium, called bullae. This chapter provides an overview of the diagnosis and management of endothelial diseases.
