Disseminated Tuberculosis Infection (Pulmonary, Peritoneal, Colonic, and Adrenal)

Tuberculosis (TB) is a significant global health problem caused by infection with Mycobacterium tuberculosis. From 1993 to 2018, there was a constant decrease in TB cases in the United States with a national incidence of 2.8 cases per 100,000 persons. Disseminated TB is rare, can affect any organ system, and presents mainly in immunocompromised populations. This study presents the case of a 37-year-old female patient with decreased consciousness, a history of chronic kidney disease (CKD) stage V, and uncontrolled diabetes mellitus lasting almost 10 years. A chest radiograph (CXR) provided the only evidence suggesting TB, leading to abdominal computed tomography (CT) and further tests showing dissemination to the pulmonary, peritoneal, colonic, and adrenal systems. The patient subsequently deteriorated, requiring ventilator support due to refractory respiratory and multiorgan failure.
