The enhanced view totally extraperitoneal (eTEP) access surgery has become one of the most reliable, reproducible, and safe surgical procedures available for the treatment of ventral hernias [3] when performed by properly trained surgeons with a clear understanding of the extraperitoneal anatomy. Originally described for groin hernias [1], the “crossover” maneuver facilitated its implementation for ventral hernias [2]. While initially described for hernias along the midline, it is currently being used to repair hernias virtually anywhere within the abdominal wall, in both simple and complex scenarios [9]. The longterm implications of this maneuver are still being studied. The eTEP access preperitoneal ventral hernia repair is a variation which aims to repair small- to medium-sized primary midline hernias with or without diastasis recti without anatomical distortion of the posterior rectus sheath attachments.
