Pediatric low-grade gliomas constitute the most common solid tumors in children. This is an umbrella term comprising various histologies, all with the underlying principle of management with surgery as primary therapy and very good prognosis. Clinical presentation and imaging criteria are characteristic but surgery extent, histologic type and molecular features including specific mutations guide need for adjuvant therapy. Role of radiation therapy is limited in primary setting but may be justifiable in large residual or recurrent tumors. Targeted therapy has a promising role in tumors with mutations in MAPK or other pathways.
