Basic Anatomy for Lifting

The facial region consists of multiple layers depending on the area, and the primary target layer varies according to the different lifting methods. Energy-based devices (EBD), such as radiofrequency and ultrasound devices, generate heat from the dermal layer of the face to the superficial musculo-aponeurotic system (SMAS) layer, causing contraction. In the case of thread lifting, the subcutaneous fat layer and SMAS layer are mechanically pulled to create a lifting effect. Surgical facelift procedures primarily involve lifting the subcutaneous fat layer, the SMAS layer, and the deeper sub-SMAS layer. Of course, excess skin is excised after the overall lifting procedure. Therefore, it is essential for the surgeon to be well aware of which layer is the primary surgical site according to the method of lifting being performed, and understanding the corresponding anatomical structures is crucial. This chapter will examine the layers of the face, the main anatomical structures located in the face, and the ultrasound detected structures.
