The cardiac shadow is the centrepiece of a chest X-ray. While the prime position in cardiac evaluation has gone to echocardiography and angiography, the plain X-ray chest is still a very valuable tool in the evaluation of cardiac-related diseases. In this section, the cardiac shadow, both normal and abnormal, as seen in the chest X-ray, will be discussed.
