An 18-year-old man presented with a 3-month history of pain and swelling over the left gluteal region. Clinical examination showed an 18 × 18 cm firm intra-muscular soft tissue mass with no overlying skin changes. MRI showed a large heterogeneous mass arising from the left gluteus maximus muscle (Fig. 1), and an 18F-FDG PET/CT scan (Fig. 2) showed an avid mass with no other sites of involvement. Core biopsy was performed which showed an embryonal rhabdomyosarcoma and bone marrow biopsy showed no disease. This was consistent with Stage III Group 1 disease. The patient underwent a wide excision of the mass, followed by adjuvant chemotherapy with Vincristine, Actinomycin D and Cyclophosphamide according to the IRS V protocol.
