A 17-year-old male presented following an injury to his right arm. He heard a crack sound after catching a basketball that was passed to him and developed immediate pain and swelling in the right arm. He presented to the Emergency Department and radiographs showed a fracture through a well-defined lytic lesion within the diaphysis of the humerus (Fig. 1). He was diagnosed with a pathological fracture through a simple bone cyst (SBC) and managed non-operatively with a splint. The patient found it difficult to tolerate the splint and opted for surgical treatment after a few weeks of conservative treatment. Open cyst curettage and stabilization with an intramedullary nail was performed, and there was complete cyst healing and fracture union with no evidence of recurrence at 2 years (Fig. 2).
