A 21-year-old male with a known history of Hereditary Multiple Osteochondromas (HMO) presented with a symptomatic left distal thigh osteochondroma (Fig. 1). While it had not been increasing in size, it caused a lot of discomfort during exercise. X-ray showed a pedunculated osteochondroma arising from the medial aspect of the distal femur, which was found to extend into the adductor canal on cross sectional imaging (CT scan shown, Fig. 2). The patient underwent an elective excision of the osteochondroma, which achieved good symptomatic relief (Fig. 3).
