This paper introduces the Life Trac Health Hub, a healthcare management system that integrates GPS and GSM technologies to monitor patients’ vital signs in real-time. It aimed at improving surveillance and health monitoring, especially for the elderly and those with chronic illnesses. A comprehensive solution for healthcare delivery and emergency response is needed, leveraging modern communication technologies for real-time updates and alerts. The proposed system employs various sensors to measure temperature, heart rate, blood pressure, and other vital parameters, with data transmitted to healthcare professionals via a GSM network for remote monitoring. The results show the efficacy of integrating all components of the proposed system. The system design, hardware implementation, data collection, and wireless communication strategies demonstrate the system’s potential to revolutionize healthcare practices by improving patient safety and promoting proactive management.
