Proximal femur sarcoma with hip joint involvement secondary to direct infiltration or a pathological neck fracture is best managed through extra-articular resection to achieve a negative margin. The hip joint is removed in continuity with the periacetabular structures. ‘Type II Pelvic resection’ can lead to a complex reconstruction to achieve a stable and functional hip. Furthermore, poor soft tissue attachment to the proximal femur prosthesis and muscle tension to provide a stable hip remains a major challenge to achieve a good functional hip.
