Hepatobiliary and Pancreatic Cancers

Cancers of the liver, biliary tract, and pancreas are difficult to treat malignancies of the abdomen and are associated with poor outcomes. In the rare instance of an early diagnosis, surgical treatment can be curative. However, the majority of cancers are diagnosed late, either as inoperable tumors or with distant metastases. While all of these cancers have been considered traditionally radioresistant, the role of radiotherapy in these cancers has evolved considerably in the last two to three decades. The advent of superior imaging, highly conformal and stereotactic delivery of higher radiation doses while sparing normal tissues and the use of advanced respiratory motion-management strategies have paved a way for the effective use of radiotherapy. In parallel, the use of targeted therapy and immunotherapy in hepatocellular cancers and the use of aggressive chemotherapy in pancreatic cancer has improved outcomes as well.
