Scoring systems play a pivotal role in neurocritical care by providing structured methodologies to assess disease severity, guide therapeutic interventions, and predict patient outcomes. This chapter delves into the evolution, applications, and limitations of widely used scoring systems in the neuro ICU, such as the Glasgow Coma Scale (GCS), FOUR) Score, and the Hunt and Hess Scale, among others. It critically examines key controversies, including the reliability and predictive accuracy of these tools, their applicability across diverse clinical scenarios, and the challenges of incorporating biomarkers and advanced technologies like machine learning. Furthermore, the chapter explores the debate surrounding the need for neuro-specific scoring systems versus adaptations of general ICU tools and discusses future directions, such as real-time integration with electronic health records and personalized scoring models.
