Anatomical Considerations of the Lumbar Spine for Ligament Reconstruction

Lumbar spinal stenosis (LSS) is a prevalent condition characterized by the narrowing of the spinal canal in the lower back, affecting millions worldwide. This study examines the anatomical considerations crucial for ligament reconstruction in LSS patients. We focus on the morphology and biomechanical properties of key ligamentous structures, particularly the ligamentum flavum (LF) and interspinous ligament. The LF exhibits significant variations across lumbar levels, with thickness increasing caudally. Its unique attachments and elastic properties play a vital role in spinal stability. The interspinous ligament, connecting adjacent spinous processes, is noteworthy for its potential as a pain generator. Our analysis reveals that precise understanding of these ligamentous structures is essential for successful surgical interventions. We discuss the implications of ligament anatomy on interspinous surgical approaches, including ligamentectomy and ligament reconstruction. The study highlights the importance of preserving spinal stability while addressing pathological changes in ligamentous structures.
