Lumbar spinal stenosis is the narrowing of the spinal canal by surrounding bone and soft tissues, which compromises neural structures. Multilevel lumbar stenoses are encountered much more frequently, driven by aging population and increased surveillance with imaging modalities. Extensive laminectomy and facetectomy often provide sufficient decompression, but the mechanical integrity can be severely impaired, especially in multilevel decompression surgery. Patients who underwent multilevel decompressive laminectomy had more complications and may require additional arthrodesis. Ligament reconstruction using interspinous ligaments can be useful in stabilization of the spine after multilevel decompression of spinal stenosis.
