Nowadays, various cutting-edge cancer immunotherapies are available to enhance the cancer treatment plan. Chimeric antigen receptor (CAR) T-cell therapy is an innovative approach that has significantly transformed cancer treatment. It harnesses the endogenous immune system of the patients to recognize and fight against the tumor cells, introducing a precision strategy to combat cancer. It provides a new opportunity for patients with inadequate choice of therapy and shows potential in personalized cancer treatments. This chapter illustrates the development of CAR T cells, from their origin to their current state, with an overview of their basic structure and evolution over time. It also summarizes the limitations and challenges associated with the therapy to provide a basic understanding so that it can be further refined and expanded to benefit more patients.
