Ovarian hyperstimulation is a well-known complication of controlled ovarian stimulation (COS). It is an iatrogenic complication, and there is no fully proven strategy to completely eliminate it. However, various strategies are available that can help reduce the incidence of OHSS. Several pharmacological and intervention strategies have been proposed to effectively reduce the risk of OHSS during and after COS. The advent of GnRH antagonist protocols combined with a GnRH agonist trigger, along with an all-freeze strategy, is among the most effective techniques to mitigate the risk of OHSS. There are certain ongoing rat model studies on newer pharmacological agents such as Nintedanib and Aflibercept, which appear to be promising alternatives in the future, although further evidence on them is awaited. This chapter aims to summarise all the cutting-edge advancements in the field of ART that can help establish an OHSS-free clinic.
