Neurotrophic Keratitis

Neurotrophic keratopathy refers to degenerative corneal epithelial breakdown resulting from impaired corneal innervation. The loss of sensation during active stages of viral infections such as recurrent HSV or VZV associated with involvement of ophthalmic division of trigeminal nerve leads to degenerative corneal changes considered to be characteristic of neurotrophic keratopathy. This results in non-healing epithelial defects, ulceration, and subsequent perforations. Though corneal innervation may be affected by a variety of etiologies, herpes simplex, and varicella-zoster keratitis are the major causes of neurotrophic keratitis. This chapter provides a detailed overview of post-herpes neurotrophic keratopathy, including its etiopathogenesis, clinical features, investigations, and management.
