Recipient Surgery in Deceased Donor Liver Transplantation

The recipient surgery in a liver transplantation is one of the most complex procedures which needs good knowledge of the surgical anatomy of the liver with regard to the portal structures and all its variations, especially the arterial anatomy and delicate dissection of the inferior vena cava along with hepatic veins. A good quality preoperative cross-sectional imaging with hepatic protocol is crucial not only to know the vital vascular anatomy and variations but also to assess portal vein thrombus, liver tumors, large portosystemic shunts, and adhesions. In deceased donor liver transplantation (DDLT), the surgical steps involve explant hepatectomy, back-table preparation of the liver allograft or benchwork, and implantation of the graft liver. The importance of proper hemostasis in all the phases of surgery cannot be overemphasized.
