Other Small Bowel Endoscopies

Push enteroscopy can be applied when small-intestine diseases are suspected in the proximal jejunum, when lesions requiring endoscopic diagnosis and biopsy are detected in capsule endoscopy or imaging tests, or when therapeutic endoscopic procedures are necessary. Push enteroscopy using colonoscopy is easy and immediately usable because it does not require special training. Intraoperative enteroscopy is indicated in cases of recurrent, severe bleeding that requires multiple transfusions or hospitalizations, when small bowel lesions identified preoperatively are difficult to access or treat with deviceassisted enteroscopy, or when lesions cannot be localized during surgical exploration. The primary indication for intraoperative enteroscopy is small bowel bleeding, but it can also be used for diagnostic and therapeutic purposes in conditions like Crohn’s disease, small bowel tumors, or polyposis. Various studies have shown that spiral enteroscopy is useful for evaluating the deep small intestine. Further, its safety, diagnostic accuracy, and therapeutic endoscopy rates are similar to those of the balloon-assisted enteroscopy. Compared with the conventional balloon-assisted enteroscopy, spiral enteroscopy has a lower procedure time and promotes better stability of endoscope manipulation. Thus, it is advantageous for performing various therapeutic interventions. The motorized spiral enteroscopy system has the benefits of the conventional manual spiral enteroscope and a remarkably high visualization rate of the whole small intestine. However, its recent withdrawal from the market due to safety concerns emphasizes the need for technological improvements before it can achieve its potential in the diagnosis and the treatment of small-bowel diseases.
