In challenging gap non-union of the tibial shaft, tibialization of the fibula (medialization of the fibula) is a useful limb salvage procedure and an alternative to amputation. High-velocity injuries may lead to massive damage to the bones and soft tissues of the leg. Persistent gap non-unions of the tibia after repeated surgeries and prolonged sepsis also result in a difficult clinical situation for the treating surgeons. Huntington (1905) conceived the concept of tibialization of the fibula which was later popularized as an alternative reconstruction to salvage such extremities. The main requirement is the availability of the required fibular segment and a vascular limb with a sensate sole.
