Could low-dose thrombolysis impact outcomes in moderate pulmonary embolism (PE)? The MOPETT trial by Sharifi et al. investigated whether reduced-dose thrombolysis improves outcomes in patients with PE. Moderate PE was defined as embolism with right ventricular dysfunction but without systemic hypotension. Patients receiving lowdose thrombolysis (50 mg tissue plasminogen activator) in addition to anticoagulation were compared to those treated with anticoagulation alone. The results showed a significant reduction in pulmonary hypertension and recurrent PE in the thrombolysis group, without an increase in major bleeding. These findings suggest that reduced-dose thrombolysis offers a safer, effective strategy for improving long-term outcomes in moderate PE, challenging the traditional reliance on anticoagulation alone.
