Procalcitonin and Antibiotic Exposure in the ICU: The PRORATA Trial

Can procalcitonin guidance reduce antibiotic consumption in ICU patients? The PRORATA trial investigated whether procalcitonin levels could guide antibiotic therapy to reduce inappropriate exposure in critically ill patients. This multicenter, randomized controlled trial enrolled ICU patients with suspected or confirmed infections and compared two strategies: procalcitonin-guided therapy versus standard care. The results showed that the procalcitonin group had a significantly shorter duration of antibiotic treatment without compromising patient outcomes, such as mortality or infection-related complications. The trial concluded that using procalcitonin as a biomarker to guide antibiotic therapy can safely reduce antibiotic exposure in the ICU, offering an effective strategy to combat antibiotic overuse and resistance. Procalcitonin levels alone have little value in the diagnosis of infection and deciding on the commencement of antibiotic therapy; however, it is likely to guide the clinician regarding the duration of therapy.
