Could delaying fluid resuscitation improve survival in penetrating trauma? This randomized controlled trial compared the effects of immediate vs. delayed fluid resuscitation in patients with hypotension following penetrating torso injuries. The study enrolled 598 patients and found that early fluid resuscitation did not improve survival rates. In fact, the delayed resuscitation group, which received fluids only after reaching the hospital, showed a slightly better survival rate (20% vs. 16%) compared to the immediate resuscitation group. The trial challenged traditional trauma care protocols, suggesting that immediate fluid resuscitation may not always be beneficial in the management of hypotensive trauma patients, particularly in those with penetrating injuries.
