Does albumin infusion play a role in sepsis management? The ALBIOS trial investigated the role of albumin in patients with severe sepsis or septic shock. This large, multicenter, randomized controlled trial (RCT) included 1800 patients who were randomized to receive either albumin (along with crystalloids) or crystalloids alone for fluid resuscitation. The study found no significant difference in 28-day mortality between the albumin group and the control group. However, patients who received albumin required less vasopressor support and experienced a shorter duration of ICU stay. A post hoc subgroup analysis also suggested improved survival in patients with septic shock with the use of albumin. This trial provided important insights into fluid resuscitation in sepsis, suggesting that while albumin did not improve survival, it may be useful in reducing vasopressor dependency.
