Cognitive impairment is a common comorbidity in older persons undergoing surgery. Such patients are at risk of developing postoperative cognitive dysfunction (POCD), leading to adverse healthcare outcomes. This chapter aims to define cognitive impairment, discuss POCD, its underlying biological mechanisms, risk factors, and precipitants, and the management of POCD. This would lead to suggestions for the prevention and amelioration of POCD in the perioperative setting and their implications for surgical perioperative care.
