This chapter proposes a holistic, patient-centered approach to measuring and improving the value in surgical care by introducing three integrated tools: the Patient Value Compass (PVC), the Patient Value Compass (PVC) score, and the Optimal Care Index (OCI). The PVC encompasses four domains—functional outcomes, clinical outcomes, patient experience outcomes, and cost-to-patient outcomes—enabling care teams to assess outcomes that matter most to patients. The PVC score aggregates these outcomes into a weighted composite index that reflects performance against team-specific baselines and targets. To measure provider value, the OCI is derived by dividing the PVC score by the cost of delivering care, offering a practical metric for evaluating efficiency in value generation. Together, these tools address the gaps in traditional metrics and can support value-based improvement across diverse care teams. The chapter also introduces the Steps of Systematic Improvement (SOSI), a four-step methodology rooted in clinical logic models, that enables care teams to systematically enhance value over time. By integrating patient perspectives, resource use, and continuous improvement methodologies, this approach offers a scalable, data-driven strategy for health systems to achieve person-centered and value-driven healthcare services.
