The Covid-19 pandemic has made it clear that the health system in most of the low- and middle-income countries (LMIC) is operating with significant shortcomings and is liable to buckle under duress. Moreover, the health system of even more developed countries has been significantly drained by the ravages of the pandemic, and a transformation is needed. Overall, the different health systems need an overhaul not only to be prepared for similar challenges of pandemics in the future but also to be more resilient for emerging diseases in a changing world. The research questions are (1) Why health system transformation is needed? (2) What are the challenges of this transformation in LMIC? and (3) What are the best ways to enact these transformative changes in an efficient and effective way? The context of health system transformation varies from region to region. Policies for change, thus, must be area specific and requires new types of leadership. The case for change must be communicated effectively to ensure support from all stakeholders. One of the major changes in the health sector will be gathering and use of data. As technology advances, collection of data and their integration will be practised on a wider scale. This new type of public health intelligence will lead to better monitoring and decision-making in the future. One example is in Uganda, event-based surveillance, especially for fevers, used all online media including WhatsApp and Twitter effectively for this purpose. Another major imminent change will be the utilization of artificial intelligence in different fields. In equitable distribution of healthcare resources, AI can play a big role. Based on incoming feedback data, AI algorithms can decide on distribution of medical supplies in a cost-effective manner. During the last pandemic, a lot of time was wasted in matching oxygen supply to the demand in Indian health sector. The use of technology can make this faster and more efficient in the future. Finally, scientific innovation will be needed to combat the emerging diseases of the future. The health system of the future must have scope for innovation from the grassroots level. Transformation of the current health system is urgently needed in coming decades. A proper planning and implementation framework must be used for the purpose.
