The Covid-19 pandemic has led to unimaginable strain on the health system as a whole, and health resource allocation has been the subject of both intense scrutiny and debate all over the world. In such unprecedented times, when demands were rising exponentially and resources were limited, a lot of patients were understandably left without adequate management and supply-demand ratio left a lot to be desired for. While there was a lack of resources in many cases, there were also some instances when sufficiently available of resources were wasted due to lack of planning. There were naturally, profound discussions on ethical, moral, and scientific implications of such allocation decisions. This chapter will discuss the root causes of the crisis in health resource allocation and the lessons learnt the hard way. The existing principles guiding resource distribution have been discussed with pros and cons of each approach. The ground level realities of prioritizing certain sections of population and addressing the pockets of “urgent need” have also been discussed. The importance of program qualities like transparency and inclusiveness has been touched upon, along with the need for regular monitoring and evaluation. The pandemic has provided an opportunity to shake up the current resource allocation system in a good way.
