The Latarjet procedure is recommended for patients presenting with humeral and/or glenoid bone loss, practicing professional activities or sports at risk of recurrence, without any proof of the superiority between the arthroscopic and the open techniques. During the open Latarjet procedure, capsular management has an important place, in particular by preventing glenohumeral arthritis that may follow this surgery. The optimal capsular management technique to use is still under debate. The purpose of this chapter is to present the open Latarjet procedure with the association of a vertical capsular shift to treat patients with an important risk of anterior shoulder instability recurrence.
